Discharge Planning
As your hospital, we realize that your health care needs extend beyond your discharge; therefore, we have provided a staff in the Social Services Department to work with you and your physician to identify and plan for your post-hospital care. Discharge planning is individualized and may include arranging for nursing or rehabilitation centers, home care nurses or therapists, equipment and transportation, or other support services.
As you are preparing for your discharge from the hospital, your physician may order home health services for you. As required by law, this hospital is providing you a list of area home health agencies from which you may choose for your home health care. This hospital does not have any relationship with any home health agencies except HomeCare. HomeCare is a division of East Ohio Regional Hospital, a sister hospital of Ohio Valley Medical Center and Peterson Rehabilitation Hospital. HomeCare is a complete home health service. It provides skilled nursing care, physical, speech and occupational therapy, home health aide care and Lifeline, a personal emergency response system for patients who live alone.
Upon your choice of a home health agency, this hospital will notify that agency which will then be responsible for making an assessment and arrangements for your home care needs. This list of available home health service providers is a Medicare program requirement and does not constitute an "implied endorsement" of any particular provider. If you believe you may need to have such arrangements made and have not had contact with a social service representative, please contact the Social Services Department at 234-8151, or ask your nurse to make the arrangements. Should you have any questions regarding any of the home health agencies, please do not hesitate to ask. Our goal is to try and make your transition to home care as smooth as possible.
